Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Experience with electronic medical charting.
• 3-5 years clinical experience.
• Flexibility with scheduling.

Responsibilities

• Develops a plan of care for each patient with input and agreement, including safety, education, and developmental needs.
• Interprets data and sets goals, revising plans as needed and communicating goals to patient/family.
• Displays expertise in recommended therapeutic procedures and modalities.
• Complies with documentation standards across professional and departmental areas.
• Addresses a home exercise program with each patient.
• Maintains an adequate caseload and adheres to productivity standards.
• Attends continuing education seminars and presents educational programs to Rehab staff or other professionals.
• Communicates with Rehabilitation Supervisor and team regarding patient care.
• All other job-related duties as assigned.
