Middletown, CT Home Care Rehab employs a Physical Therapist to administer PT services per medical prescription under CT regulations and hospital standards. The role includes clinical oversight of PTA and collaboration with the Rehab team in the home care setting.

The practitioner develops patient-centered care plans, explains goals and outcomes, maintains a consistent caseload, and participates in continuing education while aligning with supervision and physician referrals.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Experience with electronic medical charting.
• 3-5 years clinical experience.
• Flexibility with scheduling.

Responsibilities

• Develops a plan of care for each patient with input from the patient.
• Interprets data and sets goals, revising plans as needed.
• Recommends various therapeutic procedures and modalities.
• Maintains documentation as required by standards.
• Addresses a home exercise program with each patient.
• Keeps an adequate caseload and adheres to productivity standards.
• Attends continuing education and presents to Rehab staff.
• Communicates with Rehabilitation Supervisor, team, and physicians.
