Job summary

MJHS in New York, NY is seeking a Home Care Planner to evaluate and establish a plan of care for patients referred to home health services. As a key member of the business development and marketing team, you will coordinate admissions from hospital discharge and ensure care plans meet patient and family needs.

Qualifications

• Must hold a current NYS RN license and registration.
• Associates degree in nursing is required; a Bachelor's degree is preferred.
• Minimum of two years' experience in home care nursing with strong assessment and interviewing skills.
• Excellent written and verbal communication, and client-service focus.

Responsibilities

• Meets established Monthly Admission Targeted Goals.
• Delivers exceptional customer service while sustaining established business relationships and actively cultivating new ones.
• Demonstrates flexibility within the role to effectively support the agency's overall business objectives and goals.
• Responds to the referral source in a timely manner when given a patient referral.
• Establishes a plan of care in collaboration with patient, physician and interdisciplinary health care team.
• Coordinates with discharge planning staff to identify and resolve issues affecting patient transition home.
• Communicates admission criteria, reimbursement criteria and policies to patient/caregiver and health care team.
• Evaluates referral needs that align with MJHS products and services (Homefirst, Palliative Care, Hospice).
• Identifies referrals for various MJHS clinical service programs as deemed applicable.
• Ensures Home Health Face-to-Face documentation is located in Medicare patient records.
• Maintains clear, legible documentation reflecting standards and regulatory requirements.
