You will facilitate admissions from hospital referrals and collaborate with physicians, social workers, and discharge planners to ensure seamless patient care.

Qualifications

• Current NYS RN license and registration.
• Associates degree in nursing required; Bachelor's degree in nursing preferred.
• Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills.
• Client-service oriented with excellent written and verbal communication skills.
• Strong critical thinking and problem-solving abilities; able to work independently.

Responsibilities

• Evaluate and establish a plan of care for patients referred for home health services.
• Facilitate admission of referrals from hospital to a home care program that meets patient and family needs.
• Develop and maintain relationships with physicians, social workers, case managers and discharge planners within the medical center as part of the care team.
