Job summary

MJHS Hospice is seeking a Hospice Care Planner (Registered Nurse) to lead patient care planning on-site at a major medical center in New York, NY. The role focuses on evaluating care plans and coordinating admission for hospice services, while building strong relationships with clinicians and discharge staff.

Ideal candidates have at least two years in hospice nursing, NYS RN licensure, and excellent communication skills; bilingual Spanish/English is a plus in some locations.

Qualifications

• Licensed Registered Nurse with current NYS registration.
• Bachelor's Degree preferred.
• Minimum of two years hospice nursing experience.
• Strong written and verbal communication skills.
• Advanced critical thinking and problem-solving skills; able to work independently.
• Bilingual Spanish/English required in some locations.

Responsibilities

• Evaluate and establish a plan of care for patients referred for Hospice Care services.
• Facilitate admission of referrals upon discharge from the hospital to an appropriate Hospice program.
• Develop and maintain positive relationships with physicians, social workers, case managers and discharge planners within the medical center.
