Job summary

MJHS seeks a dedicated Registered Nurse for its Home Care team in Brooklyn, NY. You will lead the development of comprehensive home health plans, coordinate with physicians, and oversee patient progress to support recovery at home.

In this role you will assess patients, manage medications, educate families, and document care plans while maintaining strong communication with an integrated care team. A NYS RN license is required and a valid driver's license is essential.

Qualifications

• Current NYS RN license and registration required.
• Minimum of one year acute medical-surgical nursing experience.
• CHHA experience preferred.
• Driver license and car required.

Responsibilities

• Develop initial comprehensive home health plan of care with physician collaboration.
• Effectively case manage assigned patients to achieve positive clinical outcomes and minimize readmission risks.
• Support agency financial goals through appropriate care coordination and utilization.
• Demonstrate expert knowledge of community health nursing practices.
• Perform thorough clinical assessments and document findings accurately.
• Communicate clearly with patients, families, and the care team to ensure continuity of care.
• Collaborate with interdisciplinary team to adjust plans as patient needs evolve.
