Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Per Diem Physical Therapist to provide in-home care in Bergen County, NJ. The role offers 1099 and W-2 options, flexible scheduling, and mentorship for new graduates.

Travel to patients’ homes is required, with a focus on patient care and professional growth. Licensed Physical Therapists in NJ or those eligible to practice are welcome, with preferred certifications in First Aid, AED, and CPR.

Qualifications

• Graduate of a physical therapist program.
• Licensed physical therapist in NJ or eligible to practice.
• Preferred certifications: First Aid, AED, CPR.
• Home health experience preferred but not required; mentorship provided.

Responsibilities

• Provide 1:1 home-based physical therapy within your geographic area.
• Perform initial evaluation and develop individualized plans of care.
• Document treatment plans and progress per physician orders and agency policies.
• Educate patients and caregivers on therapy techniques and home exercises.
• Travel to patients’ homes within the assigned geographic territory.

• Compensation: USD 72 - USD 95 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Physical Therapist Program Graduate
