Ignite Rehab (DBA Metro Physical & Aquatic Therapy) seeks a part-time Home Care Physical Therapist to deliver in-home care in Ocean County, including Toms River. The role emphasizes 1:1 treatment, flexible scheduling, and collaboration with a multidisciplinary team.

Applicants should be a Physical Therapist Program Graduate and licensed or eligible to practice in New Jersey. Travel to patients' homes within the territory is required; training and mentorship provided for new grads.

Competenze

• Licensed PT in New Jersey or eligible.
• Graduate of an accredited Physical Therapist program.
• Home health experience preferred but not required; mentor provided.
• Willingness to travel to patients' homes within assigned territory.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care

• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jerseyor eligible.
• PreferredCertifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $62,000-$75,000. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
