Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated and passionate per diem Physical Therapist to provide in-home care across Bergen County, NJ. You’ll deliver 1:1 treatment in patients’ homes with flexible scheduling and travel within the assigned geographic area, including Fort Lee.

All levels of experience are encouraged to apply; we train the right new graduate. 1099 and W-2 options available, competitive salary per visit, and a supportive team of clinicians dedicated

Qualifications

• Licensed Physical Therapist in NJ or eligible.
• Physical Therapist Program Graduate.
• Home health experience preferred but not required.

Responsibilities

• Accept and treat patients within designated geographic area.
• Provide 1:1 treatment.
• Develop individualized treatment plans based on physician orders and patient needs.
• Deliver care aligned with professional standards and policies.
• Educate family members and caregivers on therapy techniques.
• Carry out occupational therapy programs to maintain or restore function.
• Perform initial exam and document findings.
• Provide timely documentation for each visit.
• Evaluate treatment effectiveness and adjust as needed.
• Willingness to treat geriatric patients.

Responsibilities:

• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• PreferredCertifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
