Metro Physical & Aquatic Therapy is seeking a motivated Per Diem Physical Therapist Assistant to provide in-home care under the supervision of a licensed Physical Therapist. This role offers flexible scheduling and per-visit compensation, with the chance to grow within a multidisciplinary team serving Bergen County families.

Must be a PTA program graduate and licensed in New York or eligible; certifications in First Aid, AED, and CPR are required.

Qualifications

• PTA program graduate.
• Licensed PTA in NY or eligible.
• Mandatory First Aid, AED, CPR certifications.
• Home health experience preferred.

Responsibilities

• Provide in-home physical therapy treatments per plan of care.

Home Care Physical Therapist Assistant- Per Diem

• Part-time
• Compensation: USD 50 - USD 60 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care
• Monitor and report changes in patient status to the supervising PT in a timely manner
• Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care
• Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT
• Assist the PT in updating care plans based on patient progress and response to treatment
• Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards
• Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT
• Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice
• Physical Therapist Assistant Program Graduate
• Licensed Physical Therapist Assistant in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
