Job summary

Ignite Rehab, operating as Metro Physical & Aquatic Therapy, seeks a motivated part-time Home Care Physical Therapist to provide in-home services in Passaic County, NJ. This role offers benefits and a flexible schedule, with 20 visits per week.

Responsibilities include 1:1 treatment, evaluation, and developing individualized plans; documentation and caregiver education; travel to patients' homes; and geriatric care as needed.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Preferred certifications: First Aid, AED, CPR Certifications.
• Home health experience preferred but not required - Mentor provided for documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

The anticipated base salary range for this position is $65,000-$75,000. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
