Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, is seeking a part-time Occupational Therapist to provide in-home care in Union County, NJ. The role offers benefits with a flexible schedule and 20 visits per week.

Qualified candidates must be licensed in NJ (or eligible), have OT program graduation, and hold First Aid/AED/CPR certifications. Travel to patients’ homes is required.

Qualifications

• Licensed OT in New Jersey or eligible.
• OT program graduate with state licensure.
• First Aid, AED and CPR certifications are required.
• Home health experience is preferred but not required; mentor provided.

Responsibilities

• Accept and treat patients within geographic area.
• Provide 1:1 occupational therapy treatment.
• Evaluate patients and develop individualized plans.
• Deliver care per professional standards and plan of care.
• Educate family and caregivers on therapy techniques.
• Plan and carry out OT programs to maintain or restore function.
• Perform initial exam and determine diagnosis before intervention.
• Provide timely documentation for each visit.
• Evaluate progress and adjust treatments; treat geriatric patients.

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New Jersey or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
