Lakeland Care Solution in Leesburg, Virginia is seeking a dedicated Home Care Physical Therapist for a full-time role. The therapist will conduct evaluations, design personalized treatment plans, and deliver evidence-based interventions in patients0homes throughout the Leesburg area.

You will work Monday to Friday, 40 hours per week, focusing on orthopedic, pediatric, neurologic and sports medicine cases while delivering education and compassionate care to improve mobility and quality of life.

Qualifikationen

• Bachelor0s degree in Physical Therapy from an accredited program required.
• State License to practice Physical Therapy and BLS CPR certification required.
• Strong communication and basic computer skills essential for patient care and documentation.
• DPT degree is advantageous and may strengthen candidacy.

• Conduct comprehensive patient evaluations and develop personalized treatment plans.
• Deliver evidence-based interventions across orthopedics, pediatrics, neurology and sports medicine.
• Educate patients and families on recovery processes and exercises for functional improvement.
• Provide excellent customer service and foster a supportive care environment.
• Collaborate with multidisciplinary teams to optimize patient outcomes and care plans.
• Monitor progress and adjust treatments based on assessments and feedback.
• Support organizational goals by meeting utilization and business objectives.
