Job summary

Elite Home Health in Leesburg is hiring a Physical Therapist (PT) to provide skilled therapy in patients' homes across Leesburg and surrounding areas. You will develop personalized treatment plans and help patients regain mobility and independence.

As part of a supportive clinical team, you will perform evaluations, supervise PTAs as required, and ensure accurate documentation while coordinating with physicians and nurses. Florida license and CPR certification are required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Florida PT license in good standing.
• Minimum 1 year of PT experience preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions.
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation.
• Communicate with physicians, nurses, and the interdisciplinary care team.
• Supervise PTAs as required.
