Job summary

Elite Home Health is hiring a Physical Therapist to deliver skilled home-based therapy across Leesburg and surrounding areas. You will evaluate patients, design individualized treatment plans, and work with a dedicated clinical team to restore mobility and independence.

This role emphasizes compassionate patient care, clinical autonomy, and opportunities for professional growth. A valid Florida PT license and CPR are required, with reliable transportation and a valid driver's license.

Qualifications

Graduate of an accredited Physical Therapy program.
Current Florida PT license in good standing.
Minimum 1 year of PT experience preferred.
Current CPR certification.
Valid driver's license, reliable transportation, and auto insurance.
Ability to meet all required background and compliance requirements.

Responsibilities

Perform physical therapy evaluations and develop individualized treatment plans.
Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions.
Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
Monitor patient progress and adjust treatment plans as needed.
Complete timely and accurate clinical documentation.
Communicate with physicians, nurses, and the interdisciplinary care team.
Supervise PTAs as required.
