Job summary

Elite Home Health is seeking a Physical Therapist to provide skilled services in patients' homes across Leesburg and surrounding areas. You will conduct evaluations, design individualized plans, and work one-on-one with patients to restore mobility and independence.

The role requires a Florida PT license, CPR certification, and at least 1 year of PT experience, with a valid driver's license for home visits. You'll collaborate with physicians and the care team to ensure high-quality care.

Qualifications

Graduate of an accredited Physical Therapy program.
Current Florida PT license in good standing.
Minimum 1 year of PT experience preferred.
Current CPR certification.
Valid driver's license, reliable transportation, and auto insurance.
Ability to meet all required background and compliance requirements.

Responsibilities

Perform physical therapy evaluations and develop individualized treatment plans.
Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions.
Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices.
Monitor patient progress and adjust treatment plans as needed.
Complete timely and accurate clinical documentation.
Communicate with physicians, nurses, and the interdisciplinary care team.
Supervise PTAs as required.
