Job summary

Lakeland Care Solution in Leesburg, Virginia is seeking a dedicated Home Care Physical Therapist for a full-time role. The therapist will conduct evaluations, design personalized treatment plans, and deliver evidence-based interventions in patients0homes throughout the Leesburg area.

You will work Monday to Friday, 40 hours per week, focusing on orthopedic, pediatric, neurologic and sports medicine cases while delivering education and compassionate care to improve mobility and quality of life.

Qualifications

• Bachelor0s degree in Physical Therapy from an accredited program required.
• State License to practice Physical Therapy and BLS CPR certification required.
• Strong communication and basic computer skills essential for patient care and documentation.
• DPT degree is advantageous and may strengthen candidacy.

Responsibilities

• Conduct comprehensive patient evaluations and develop personalized treatment plans.

Work Environment

As a Home Care Physical Therapist, you will work onsite in patients' homes throughout the Leesburg area. Key Responsibilities

• Engage in effective communication with patients and their families, providing education on recovery processes and techniques for functional improvement.

• Collaborate with multidisciplinary healthcare teams to optimize patient outcomes through coordinated care and shared decision-making.
• Monitor patients’ progress closely and make necessary adjustments to treatment plans based on ongoing assessments and patient feedback.
• Meet established utilization and business objectives to ensure the success and sustainability of the practice.

Patient Population / Client Focus

This role will primarily focus on a diverse patient population, including orthopedic, pediatric, neurologic, and sports medicine cases. You will have the opportunity to inspire and motivate patients of all ages and backgrounds, helping them to reclaim their mobility and independence. As you develop treatment plans, your emphasis will be on nurturing relationships that enhance patient adherence and satisfaction.

Qualifications

• A Bachelor’s degree in Physical Therapy from an APTA or CAPTE-approved program is required.
• A valid State License in Physical Therapy and certification in Basic Life Support from the American Heart Association.
• Proficiency in written and verbal communication along with competency in basic computer applications.
• A strong commitment to patient-centered care, demonstrating the ability to build rapport and trust with patients and their families.

Preferred Experience

• Two years of clinical experience in physical therapy is preferred, showcasing expertise in evaluating and treating a range of conditions.
• Obtaining a Doctorate of Physical Therapy (DPT) is highly advantageous and may further enhance your candidacy.
• Current CPR certification is required to ensure patient safety in all scenarios.
