Middletown Home Care Rehab is seeking a licensed Physical Therapist to provide patient-centered therapy in a home care setting. The role requires evaluating patients, developing individualized care plans, and supervising treatment under state regulations.

The therapist will maintain caseloads, document care per standards, and participate in continuing education. 32 hours per week during dayshift at our Middletown, CT site.

Qualifikationen

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in Connecticut.

• Interprets patient data and updates goals as needed, communicating with patient/family.
