Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Preferred: Experience with electronic medical charting.
• 3-5 years clinical experience.
• Flexibility with scheduling.

Responsibilities

• Develops a plan of care for each patient with input and agreement.
• Interprets and assesses data about the patient with goal setting and revision of goals.
• Recommends various therapeutic procedures and modalities.
• Maintains documentation per standards.
• Addresses a home exercise program with each patient.
• Maintains an adequate caseload and adheres to productivity standards.
• Attends continuing education seminars and presents educational programs.
• Communicates with Rehabilitation Supervisor and physicians.
