Job summary

Middlesex Health in Middletown, CT is seeking a Physical Therapist to provide Home Care Rehab services under medical prescription. You will develop plans of care, educate patients and families, and coordinate with Rehabilitation Supervisor and treating physicians.

This role requires CT licensure or eligibility, a PT degree, EMR experience, and 3–5 years of clinical practice. 32 hours per week, day shift, with a comprehensive benefits package.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in Connecticut.
• Experience with electronic medical charting (EMR).
• 3-5 years of clinical experience.
• Flexibility with scheduling.

Responsibilities

• Develops a plan of care for each patient with input and agreement.
• Interprets and assesses data about the patient; sets and revises goals.
• Recommends therapeutic procedures and modalities.
• Maintains documentation standards.
• Addresses a home exercise program with each patient.
• Maintains an adequate caseload and adheres to productivity standards.
• Attends continuing education and presents educational programs.
• Communicates with Rehabilitation Supervisor and team.
• All other job related duties as assigned.
