Job summary

Middlesex Health is seeking a Physical Therapist to provide services under medical prescription in a home care rehab setting in Middletown, CT. You will oversee PT services, develop care plans, and communicate with the care team and family.

The role requires graduation from an accredited PT program and CT licensure eligibility, with preferred experience in electronic medical charts and 3–5 years of clinical practice. A flexible schedule is valued as patient needs dictate.

Qualifications

• Experience with electronic medical charting is preferred.
• 3–5 years clinical experience preferred.
• Flexibility with scheduling is preferred.

Responsibilities

• Develops a plan of care for each patient with input and agreement, including safety and educational needs.
• Interprets data and sets goals, explains goals and outcomes to patient/family.
• Recommends therapeutic procedures and modalities.
• Maintains documentation standards.
• Addresses a home exercise program for each patient.
• Maintains caseload per productivity standards.
• Attends continuing education and presents to staff.
• Communicates with Rehabilitation Supervisor and physicians.
• Performs other duties as assigned.
