Job Summary

Reporting to the Rehabilitation Team Lead, the Physical Therapist is responsible for comprehensive patient management including evaluation, diagnosis, intervention, and patient/family education for developing and implementing individualized care plans in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson’s Disease, Covid Recovery, and Post-Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Physical Therapy, with opportunities for growth through education, in-services, and care management groups. This environment offers family-friendly, flexible hours.

Minimum Qualifications

• Education: Bachelor's Degree or higher from an accredited physical therapy program.
• Experience: Prior Home Care experience preferred, or at least one year of experience in an acute and/or rehab setting.
• Licensure and/or Certification: MA PT license and BLS required within 30 days of hire.
• Skills: Clinical expertise, workload prioritization, communication, assessment, critical thinking, organization, problem-solving, computer skills, confidentiality, and proficiency in English. Commitment to Service Excellence and customer satisfaction.
