Job Summary

Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.
Qualifications
Key Responsibilities
- Develop a plan of care that is patient and family focused and institutes standardized interventions for goal oriented outcomes
- Daily documentation is accurate and timely
- Application of standard treatment procedures
- Teaching is provided to patients and caregivers
- Age appropriate evaluation is completed that reflects patient history, relevant systems review and appropriate tests and measures and reported to primary clinician.
- Plan of treatment is reassessed at least every 2 weeks to patient, primary caregivers and interdisciplinary team; plan is modified and communicated as appropriate to meet discharge goals.
- Physician orders are current and consistent with plan of care and documented in the medical record.
- Patient schedules are submitted to the rehab scheduler by end of day Thursday.
- PTAs are utilized as directed by the Manager to optimize capacity and ensure compliance with visit schedules.
- Patient caseloads of Physical Therapy Assistants will be managed in accordance with agency, state and professional guidelines.
- Home Health aides will be oriented and supervised in accordance with agency and state guidelines.
- Documentation is complete and submitted timely according to agency standards.
- POC devices are updated weekly and as directed
- Verification of Payor information on admission is verified in computer system and attended according to agency standards.
- Documentation to support Managed Care authorizations/DME requirements is maintained/transmitted within designated time frames.
- Visit schedule is consistent with authorizations by Managed Care payors.
- Utilization/Caseload is discussed with Clinical Manager on an ongoing basis and changes are made toward stated goals.
- Reportable outcomes meet Agency goals.
- . Realistic, objective, time oriented goals for patient performance are identified based on examination findings.
- Treatment and discharge plans are selected based on examination findings and patient goals.
- Patient is informed of his/her weekly schedule and any other changes to their schedule as necessary.
- Receives positive comments from patients/families/referral sources/co-workers/management.
Licensure, Certifications or Registration
Required: Registered with the Board of Registration in Massachusetts.
Experience
- One year experience in acute care, rehab or homecare as a Physical Therapist
- Registered with the Board of Registration in Massachusetts.
- Strong customer service skills and ability to work with diverse populations
- Excellent verbal, written and electronic communications skills
- Computer proficiency and ability to learn new software
- Ability to prioritize work and adapt to changing situations
- Travel required within MGB Home Care’s geographic area
