Summary
  • The Physical Therapist assesses the physical abilities of patients, restores maximum function, maintains patient capabilities, relieves pain, prevents disability or deformity, and develops or improves physical abilities.
Responsibilities
  • Provides skilled physical therapy to patients in the home.
  • Reviews patient progress with other disciplines involved, at least every 14 days.
  • Communicates with the team leader, social and government agencies, and physicians as needed.
  • Assesses patient needs, home environment, medical equipment requirements, and medical/social needs.
  • Develops and updates care plans based on assessments.
  • Provides direct skilled therapy and instructs patients on exercises and care.
  • Supervises home health aides biweekly when applicable.
  • Prepares patients and caregivers for discharge, documents all care and assessments timely.
  • Schedules patient visits and follows physician orders for treatment plans.
  • Participates in performance improvement, team meetings, and mandatory training.
Credentials
  • Basic Cardiac Life Support (Required)
  • Driver's License and Car Insurance (Required)
  • Licensed as a Physical Therapist in Pennsylvania (Required)
Education or Equivalent Experience
  • Bachelor of Arts or Science (Required)
  • 1+ years of home health physical therapy experience preferred
