Summary:

  • The Physical Therapist assesses the physical abilities of patients, restores maximum function, maintains patient performance, relieves pain, prevents further disability or deformity, and develops or improves physical abilities.

Responsibilities:

  • Provides skilled physical therapy to patients in the home.
  • Manages home health when a primary nurse is not involved, reviews patient progress with other disciplines, and communicates with team members and external agencies as needed.
  • Assesses patient needs, home environment, and medical equipment requirements.
  • Contacts physicians regarding patient status and orders, and reports changes or issues promptly.
  • Develops and updates care plans, instructs patients and caregivers, and provides direct therapy.
  • Supervises home health aides, reviews and updates care plans, and discusses care with patients and families.
  • Manages discharge planning and documentation, schedules visits, and participates in quality improvement activities.
  • Ensures compliance with all documentation and regulatory requirements.

Credentials:

  • Current Pennsylvania Physical Therapist license
  • Basic Cardiac Life Support (BLS) certification
  • Valid driver’s license, car insurance

Education or Experience:

  • Bachelor of Arts or Science degree (required)
  • At least 1+ years of home health physical therapy experience preferred
