Job summary

Liberty Home Care is seeking an experienced Home Care Occupational Therapist (OT) Per Diem, PRN Flexible to serve Onslow, Jones, Craven and Carteret Counties in North Carolina. The OT will provide patient care, perform evaluations, and develop and monitor plans of care in collaboration with COTAs.

The role requires a bachelor’s degree in occupational therapy, NBCOT licensure, CPR certification, and strong communication and organizational skills.

Qualifications

• Bachelor’s degree in occupational therapy and current state licensure.
• Successful completion of the National Certification Exam (NBCOT).
• CPR certification and strong communication skills.
• Minimum of twelve hours of continuing education annually.
• Willing to travel as assigned.
• Prefer at least one year experience in non-home health setting.

Responsibilities

• Provides patient care services with direct treatment and follow-up.
• Evaluates new patients using motor, sensory-perceptual-cognitive, and performance evaluations.
• Develops a plan of care based on assessment findings.
• Establishes measurable goals aligned with patient assessments.
• Documents patient care data reflecting progress.
• Coordinates with COTAs and supervises their patient care monthly.
• Communicates with Scheduler and care team.
• Discusses treatment plans with patient/family and care coordinator.
• Coordinates referrals to appropriate disciplines.
• Provides in-service education and supports training of staff.
• Assists with orientation of new employees.
