Job summary

Liberty Health in Durham, NC is seeking a Home Care Occupational Therapist (OT) to provide direct patient care in a home health setting on a per diem, PRN basis. You will evaluate patients after illness or surgery, develop and monitor a personalized plan of care, and collaborate with COTAs to supervise care.

You will document progress, set measurable goals, and coordinate with the Patient Care Coordinator and physicians. Travel to patient homes is required as assigned.

Qualifications

• Must hold a Bachelor's degree in Occupational Therapy with NBCOT certification.
• Current licensure to practice in the state where therapy is provided.
• CPR certification and strong communication and organizational skills.

Responsibilities

• Provides patient care services with direct treatment and follow-up.
• Evaluates new patients using motor, sensory-perceptual, cognitive, and performance assessments as indicated.
• Develops a plan of care based on assessment findings.
• Sets measurable goals aligned with the patient’s physical assessment.
• Documents patient care data thoroughly and timely, showing progress.
• Coordinates with COTAs and provides monthly supervision of their patients.
• Schedules daily/weekly patient visits with the Scheduler.
• Communicates with intercompany contacts and discusses reimbursement policies with patient/family.
• Coordinates referrals to appropriate disciplines with the Patient Care Coordinator.
• Provides in-service and peer supervision and supports training as needed.
• Assists with orientation of new employees and conducts relevant research.
