Job summary

Hireology is seeking a Home Care Occupational Therapist to deliver skilled OT services in patients' homes, evaluating needs and creating individualized treatment plans to promote independence in ADLs and mobility.

You will collaborate with physicians, nurses, and other care professionals, educate patients and families, document progress, and travel within the service area. A state OT license and NBCOT are required, with 1 year of home health experience preferred.

Qualifications

Responsibilities

• Conduct comprehensive in-home assessments of patients’ physical, cognitive, and functional abilities.
• Develop and implement individualized occupational therapy treatment plans.
• Provide therapeutic interventions to improve ADLs, mobility, and safety.
• Train patients and caregivers on adaptive techniques, equipment use, and home modifications.
• Recommend and assist with adaptive equipment and devices.
• Evaluate home environments for safety hazards and modifications.
• Document evaluations, progress notes, and discharge summaries per standards.
• Collaborate with physicians, nurses, and other care team members.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and families on condition management and injury prevention.
• Participate in care coordination and interdisciplinary team meetings.

Position Summary

The Home Care Occupational Therapist provides skilled occupational therapy services to patients in their homes. The role focuses on helping individuals improve or regain independence in daily living activities following illness, injury, surgery, or chronic conditions. The therapist evaluates patient needs, develops individualized treatment plans, and works collaboratively with physicians, nurses, and other healthcare professionals to promote optimal functional outcomes in a home-based setting.

Key Responsibilities

• Conduct comprehensive in-home assessments of patients’ physical, cognitive, and functional abilities

• Provide therapeutic interventions to improve activities of daily living (ADLs), mobility, and safety
• Train patients and caregivers on adaptive techniques, equipment use, and home modifications
• Recommend and assist with procurement of adaptive equipment and assistive devices
• Evaluate home environments for safety hazards and suggest modifications
• Document evaluations, progress notes, and discharge summaries in accordance with agency and regulatory standards
• Collaborate with physicians, nurses, physical therapists, and other care team members
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and families on condition management and injury prevention
• Participate in care coordination and interdisciplinary team meetings

Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program
• Current state Occupational Therapist license (required)
• NBCOT certification (required, if applicable by state)
• Valid driver’s license and reliable transportation for home visits
• Minimum 1 year of clinical experience preferred (home health experience strongly preferred)
• Strong knowledge of rehabilitation techniques and adaptive equipment
