Job summary

Alpha Home Health and Hospice in Ocala, FL seeks an Occupational Therapist to assess, teach, and deliver home-based therapy per physician-directed plans. You will collaborate with the inter-disciplinary team to set goals and plan care that improves patient well-being.

You will conduct assessments, develop evaluations, deliver interventions, document progress, and participate in discharge planning with patients and families. Preference for home health and OASIS documentation experience.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted OT license in practice state.
• Valid CPR, driver’s license, and proof of auto insurance.

Responsibilities

• Assess mobility, strength, and functional capacity.
• Deliver therapy services per physician orders and modify plans as needed.
• Develop written evaluations and plans of care outlining OT needs and goals.
• Discharge planning and caregiver education for home exercises.

Position Overview

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with interdisciplinary teams to optimize patient well‑being through evaluations, goal setting, and treatment planning.

Responsibilities Include

• Conduct initial and ongoing assessments of mobility, strength, and functional capacity.
• Administer and interpret tests to establish functional goals and outcomes.
• Develop written evaluations and plans of care outlining occupational therapy needs, progress, and expected results.
• Deliver therapy services per physician orders, documenting performance and modifying treatment plans as needed.
• Manage discharge planning by educating patients and caregivers on home exercises and post-discharge care.
• Coordinate services with healthcare team members, including physicians and caregivers.
• Maintain accurate documentation to ensure compliance and prevent claim denials.
• Communicate effectively with staff, patients, and families, serving as a clinical resource.
• Supervise other personnel (e.g., OT Assistants, Home Health Aides) as applicable.
• Complete OASIS assessments within scope of practice.
