Job summary

LiveWell Homecare Agency, LLC seeks an Occupational Therapist to provide skilled OT services in a home health setting. You will evaluate patients, create treatment plans, deliver interventions, and collaborate with families and healthcare teams to support discharge needs and equipment requirements.

You will educate patients and caregivers, document services per policies, delegate tasks to assistants, and coordinate referrals as needed in a PRN, non-benefit-eligible role with potential mileage

Qualifications

• OT license and valid driver's license required.
• Bachelor’s degree in Occupational Therapy; Master’s preferred.
• No prior experience required; supervision not mandatory.
