Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist to provide direct patient care in a home environment. You will assess, develop and adjust treatment plans, and educate patients and families on therapy activities and equipment use.

You will collaborate with physicians and the healthcare team to maximize patient outcomes, re-evaluate progress, and ensure integrated care across services. A valid OT license and driver's license are required, and opportunities for professional

• Valid OT license and a valid driver's license are required.
• No prior experience is required for this role.
• Strong interpersonal and instructional skills are essential.

Responsibilities

• Conduct assessments and develop treatment plans for patients at home.

• Re-evaluate and modify treatment plans based on progress.
