Hospice Savannah, Inc. is seeking a Home Care Clinical Coordinator to provide administrative support across the Home Care department.

The role focuses on coordinating day-to-day operations, payroll processing, staff scheduling, documentation, and IDT meeting preparation to support high-quality patient care in a compassionate hospice setting. Reporting to the Home Care Manager, this full-time, non-exempt position requires strong organizational skills, professional communication, and the ability

Qualifications

• High school diploma or equivalent required.
• Minimum of three years of experience in general office work.
• Proficiency in Microsoft Office products, including Word, Excel, and Outlook.
• Experience with Electronic Medical Record (EMR) systems and database management.
• Experience using a multi-line phone system.
• Valid drivers license, automobile insurance, and reliable transportation required

Responsabilités

• Coordinate day-to-day departmental operations for Home Care, including payroll processing, staff scheduling, and documentation management.
• Prepare IDT (interdisciplinary team) meeting materials and ensure timely communication among staff, patients, and families.
• Collaborate with clinical and non-clinical team members to uphold service excellence and standards of Hospice Savannah.
• Maintain professional boundaries and effective communication across workplace relationships in a healthcare environment.
