Job summary

Ignite Rehab, operating as Metro Physical & Aquatic Therapy, seeks a per diem Occupational Therapist for in-home care in Union County, NJ. Flexible scheduling and a range of work arrangements are offered, including 1099 and W-2 options with competitive pay per visit.

You will provide 1:1 OT services, travel to patient homes, and collaborate with a professional team to deliver high-quality care and improve patient outcomes.

Qualifications

• Licensed Occupational Therapist in New Jersey or eligible.
• OT program graduate with required licensure.
• First Aid, AED and CPR certifications.
• Willingness to travel to patients’ homes within the assigned geographic area.
• Strong communication with patients, aides, and management.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
