Ignite Rehab, operating as Metro Physical & Aquatic Therapy, seeks a per diem Occupational Therapist to perform in-home care in Union County, NJ. The role supports a flexible schedule with 1099 or W-2 options and a focus on patient-centered outcomes.

Licensed OT in New Jersey or eligibility is required, with mandatory First Aid, AED, and CPR certifications. Prior home health experience is preferred; mentorship will be provided to ensure documentation and compliance.

Responsabilidades

• Accept and treat patients within your designated geographic area.
• Provide 1:1 occupational therapy in home settings.
• Evaluate patients and develop individualized treatment plans according to physician orders.
• Deliver care that aligns with professional standards and plan of care.
• Educate family members and caregivers on therapy techniques.
• Plan and carry out OT programs to maintain or improve function and prevent dysfunction.
• Perform initial exam and document findings prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate treatment effects and adjust plans to maximize benefit.
• Willingness to treat geriatric patients.

• Compensation: USD 70 - USD 75 - hourly

Responsibilities:

• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New Jersey or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
