Job summary

BrightStar Care of Salt Lake City is seeking a dedicated Registered Nurse to provide in-home care in Park City and surrounding areas. You will work per shifts listed, delivering high-quality, patient-centered care directly in clients' homes.

Responsibilities include medication oversight, assessments, and collaboration with families, with a focus on neurological and behavioral health needs. Join a team committed to compassionate, professional home health services and reliable scheduling.

Qualifications

• Must have at least 1 year nursing experience with neurological and/or behavioral health.
• Must have at least 1 year nursing experience in home care.
• Current unrestricted RN license in Utah and CPR certification.

Responsibilities

• Provide thoughtful care to clients based on personalized Plan of Care.
• Provide in home assessments.
• Monitor and track changes in the client's conditions and inform Directors/appropriate personnel.
• Collaborate with the client’s family on plan of care, health changes, and concerns.
• Perform total personal cares.

Client is a 19-year old male with TBI. He is wheelchair bound, limited speech, hemiplegic, neurological and/or behavioral health experience a must.

Available Shifts in Park City:

• Monday - Thursday 2:30pm - 7pm
• Friday, Saturday and Sunday 1pm - 7pm
• Saturday, Sunday & Monday 7pm - 7am

We are looking for an RN who is interested in the following:

• Medication set up and oversight

Responsibilities:

• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Total personal cares

RN Requirements

• Must have at least 1 year nursing experience with neurological and/or behavioral health
• Must have at least 1 year nursing experience in home
• Current unrestricted RN license in Utah
• Certified CPR Certification
• Negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
• Must be able to lift 75 pounds
