Job summary

BrightStar Care of Salt Lake City is seeking a licensed RN for Park City home health shifts. Deliver skilled nursing care, medication setup, and client assessments in homes, collaborating with families on plans of care.

The role requires 1 year of neurological and home-health experience, Utah RN license, CPR, TB test, and the ability to lift 75 pounds. Weekly pay with benefits for part- and full-time, and a supportive team environment.

Qualifications

• Must have at least 1 year nursing experience with neurological experience.
• Must have at least 1 year nursing experience in home.
• Current unrestricted RN license in Utah.
• Certified CPR Certification.
• Negative TB skin test or chest x-ray.
• Must be able to pass a drug test and a background check.
• Licensed driver with valid automobile insurance.
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
• Must be able to lift 75 pounds.

Responsibilities

• Medication set up and oversight.
• Provide thoughtful care to clients based on personalized Plan of Care.
• Provide in home assessments.
• Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed.
• Collaborate with client's family on plan of care, health changes, and concerns.
• Total personal cares.

Client is a 21-year old male with a TBI. He is wheelchair bound, limited speech, hemiplegic, neurological and/or behavioral health experience a must.

Available Shifts in Park City

• Monday - Thursday 2:30pm - 7pm
• Friday, Saturday and Sunday 1pm - 7pm
• Saturday, Sunday & Monday 7pm - 7am

We are looking for an RN who is interested in the following

• Medication set up and oversight

Provide thoughtful care to clients based on personalized Plan of Care

Provide in home assessments
