Pay Range: $19.50 - $23.50 per hour (based on previous work experience)
Schedules: Schedule: Full-Time|80 hours per pay period | Days will vary and includes occasional weekends and holiday rotation
Travel: Requires travel to participant homes.
The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant’s in living independently in the community. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of the Interdisciplinary Team. This position may require travel to participant homes.
Senior Community Care of Colorado PACE (Montrose) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs. Located in beautiful Montrose, the community is surrounded by stunning Colorado scenery, including the Black Canyon of the Gunnison National Park to the east and the San Juan Mountains to the south, with easy access to a charming downtown, local dining, and year-round outdoor recreation.
