Job summary

Senior Community Care of Colorado PACE (Montrose) is looking for a Med Room-LPN to join our on-site team. You will provide nursing care to our elderly participants in a compassionate, mission-driven environment, coordinating with an interdisciplinary team to deliver high-quality, person-centered care.

The requirements include graduation from a nursing program, CPR certification, and at least one year of experience with frail or elderly populations; IV certification is preferred.

Qualifications

  • Graduate of a school of professional nursing is required.
  • CPR certification required.
  • Minimum of one year experience with frail or elderly population.
  • IV certification preferred.
  • Must have a valid driver’s license, proof of insurance and transportation.

Responsibilities

  • Performs initial assessments and annual reassessments as part of the Interdisciplinary Team.
  • Administers medications and implements care orders per plan.
  • Provides education to participants and caregivers about plan of care.
  • Communicates participant changes to primary care and coordinates 24-hour care delivery.
  • Maintains participants’ medical records and charting per policy.
  • Triages participants to determine priority of care.
  • Instructs on medications and treatment instructions.
  • Supports care education for frail and elderly as requested.
  • Performs other duties as required.

Pay Range: $29 - $31.03 per hour (based on experience)
Schedule:
Monday - Friday 8am-5pm, occasional weekends and on-call rotations

What You Bring (Requirements)

  • Graduate of a school of professional nursing required.
  • Cardiopulmonary Resuscitation (CPR) certification required.
  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Intravenous Venous (IV) Certification preferred.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For

The LPN uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care and contributes to the evaluation of individualized interventions related to the plan of care. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regard to plan of care, nursing treatments or medications. Communicates participant’s condition to primary care givers, physician and interdisciplinary team.

  • Uses and practices basic assessment and assists Clinic Supervisor as designated to complete initial assessment of new PACE enrollee and annual reassessment.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Performs basic nursing services as delegated by Clinic Supervisor.
  • Performs the administration of prescribed medications and treatments in accordance with nursing standards.
  • Maintains participants’ medical record and fulfills Senior CommUnity Care charting and reporting requirements.
  • Triages participants to determine priority of care.
  • Instructs participants and family regarding medications and treatment instructions and care plan.
  • Provides education and training on care and treatment issues of the frail and elderly as requested.
  • Performs other duties as required or requested.
