Job summary

Anuco Home Health Care LLC is seeking a compassionate Medical Social Worker to provide intermittent psychosocial services in patients' homes in Chicago, IL. The role collaborates with physicians, nurses, therapists, caregivers, and the interdisciplinary team to address barriers affecting health and safety at home.

The ideal candidate has home health experience, strong knowledge of community resources, and proficiency with Axxess Home Health EMR, with a proven record of accurate documentation.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Current Illinois social work license (LSW or LCSW).
• At least 1 year of social work experience in a healthcare setting (home health preferred).
• Axxess Home Health EMR experience preferred.
• CPR/BLS certification when required by policy.

Responsibilities

• Complete psychosocial assessments per physician orders and plan of care.
• Identify social, emotional, financial, environmental, and caregiver needs.
• Assist team in understanding factors affecting patient health and safety.
• Develop and update the individualized plan of care.
• Set measurable psychosocial goals and outcomes.
• Provide short-term counseling, crisis intervention, and support services.
• Assist with advance-care and long-term care planning and caregiver support.
• Connect patients with Medicaid, Medicare, and community resources.
• Coordinate referrals for available assistance programs.
• Support discharge planning and safe transitions.
• Maintain relationships with community agencies and healthcare providers.
• Report changes in patient condition and risks to the RN and Clinical Manager.
• Participate in team meetings, QA activities, and training.
