Job summary
Anuco Home Health Care LLC is seeking a compassionate Medical Social Worker to deliver intermittent psychosocial services in patients’ homes in the Chicago area. You will assess barriers, collaborate with the interdisciplinary team, and help coordinate community resources to support safe, independent living at home.
The ideal candidate has an MSW, an IL license (LSW/LCSW), and at least one year of healthcare social work experience, preferably in home health.
Qualifications
- Master’s degree in social work from a CSWE-accredited program.
- Current Illinois social work license (LSW or LCSW) required.
- At least one year of healthcare social work experience preferred.
- Home health experience strongly preferred.
- Axxess Home Health EMR experience preferred.
- CPR/BLS certification as required by agency policy.
- Valid driver's license and reliable transportation.
Responsibilities
- Complete psychosocial assessments per physician orders and plan of care.
- Identify social, emotional, financial, and environmental needs.
- Assist team in understanding factors affecting patient health and safety.
- Develop and update individualized plans of care.
- Establish measurable psychosocial goals and outcomes.
- Provide short-term counseling and crisis intervention.
- Coordinate with Medicaid, Medicare, and community resources.
- Document referrals, resources, and care coordination in EMR.
