Job summary

BoldAge PACE is seeking a Team Assistant to support the Interdisciplinary Team (IDT) in Lakehurst, NJ. This full-time role focuses on coordinating care updates, documentation, and ensuring services are delivered as ordered in care plans.

The role requires at least a high school diploma and 1+ years of clerical experience, with preference for those who have worked with a frail or elderly population. A strong, organized, patient-centered mindset is essential.

Qualifications

• High School Diploma or related required.
• 1+ years clerical experience, preferably in a medical setting.
• 1 year experience with frail/elderly population preferred; training may be provided.

Responsibilities

• Conduct daily chart audits to verify services ordered are completed and followed up by the appropriate discipline.
• Attend IDT meetings, take notes and facilitate as needed.
• Assist care team manager with grievance resolution and appeals.
• Participate in quality initiatives as assigned.
• Answer calls from participants on their IDT panel; route and schedule using EMR.
• Assist with obtaining consultations, external records and enter into EMR.
• Other duties as assigned.
