MySolutions Home Care, part of Lutheran Life Communities, is seeking a Home Care Coordinator in Arlington Heights. The role involves managing intake and care coordination for clients, with a schedule Monday–Friday, 8:00 a.m.–4:00 p.m., and occasional evenings and weekends. You will visit residents, assess needs, develop service plans, and coordinate home care and handyman services while building strong relationships with clients and families.

Qualifications

• High school diploma or equivalent.
• Valid driver's license for regular home visits.
• Able to read, write, and comprehend English instructions.
• Basic math and computer proficiency.

Responsibilities

• Manage intake calls and follow up on incoming leads for MySolutions Home Care services.
• Visit new residents after admission and again before discharge to support smooth care transitions.
• Assess client needs and develop individualized service plans.
• Oversee delivery of home care and handyman services.
• Conduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments.
• Build strong relationships with clients, families, and caregivers, and connect them with community resources.
• Assist with caregiver scheduling, onboarding, and training.
