MySolutions Home Care, part of Lutheran Life Communities, is seeking a Home Care Coordinator in Arlington Heights. The role involves managing intake and care coordination for clients, with a schedule Monday–Friday, 8:00 a.m.–4:00 p.m., and occasional evenings and weekends. Responsibilities

• Manage intake calls and follow up on incoming leads for MySolutions Home Care services.
• Visit new residents after admission and again before discharge to support smooth care transitions.
• Assess client needs and develop individualized service plans.
• Oversee delivery of home care and handyman services.
• Conduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments.
• Build strong relationships with clients, families, and caregivers, and connect them with community resources.
• Assist with caregiver scheduling, onboarding, and training.

MySolutions Home Care, part of Lutheran Life Communities, is hiring a Home Care Coordinator in Arlington Heights. $20.00–$21.00/hour, credit for experience. Full-time, Monday–Friday, 8:00 a.m.–4:00 p.m., with occasional evenings and weekends.

As a Home Care Coordinator, you'll manage intake and care coordination for clients receiving MySolutions Home Care services, helping residents and families navigate care transitions smoothly, from initial intake through ongoing home visits.

What you'll do

• Manage intake calls and follow up on incoming leads for MySolutions Home Care services
• Visit new residents in MyRehab shortly after admission and again before discharge to support smooth care transitions
• Assess client needs and develop individualized service plans
• Oversee delivery of home care and handyman services
• Conduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments
• Build strong relationships with clients, families, and caregivers, and connect them with community resources
• Assist with caregiver scheduling, onboarding, and training

What you'll need

Required:

• High school diploma or equivalent

Preferred:

• College degree
• Background in healthcare, social work, sales, or senior living
• Supervisory experience
