Qualifications

• Previous experience in home care, healthcare, senior living, case management, social services, or care coordination.
• Strong interpersonal and relationship-building skills.
• Excellent organization and time-management abilities.
• Ability to work independently while collaborating with a team.
• Comfortable using technology and documentation systems.
• Valid driver's license and reliable transportation required.
• Bachelor's degree preferred.

Responsibilities

• Conduct in-home client assessments and reassessments; build relationships with clients and families.
• Monitor client satisfaction and address service concerns promptly; collaborate with scheduling teams.
• Support care plan implementation and ongoing service delivery.
• Serve as primary contact for assigned caregivers; conduct check-ins and performance discussions.
• Assist with caregiver matching and retention; engage in community outreach and referrals.
• Maintain accurate client and caregiver documentation; track activities in systems.
• Partner with recruiting, scheduling, and leadership to support growth goals.
