Home Care Coordinator, you’ll play a key role in ensuring that every client receives exceptional, personalized care. You’ll manage caregiver scheduling, coordinate services, and help maintain the highest standards of client satisfaction and quality care. Scheduling & Staffing.

• Coordinate daily and weekly caregiver schedules to ensure all shifts are filled accurately and on time.

• Match caregivers to clients based on skills, experience, and personality fit.

• Proactively address open shifts, attendance issues, and last-minute changes.

• Maintain real-time accuracy in our scheduling system.

Client & Caregiver Support

• Serve as the primary point of contact for clients and caregivers regarding schedules and service needs.

• Build and nurture strong relationships that promote client satisfaction and caregiver retention.

• Perform client intakes and coordinate assessments and reassessments with RNs and the Director of Nursing.

• Collaborate with the care team to ensure each client’s plan of care is followed and updated as needed.

• Participate in on-call rotation to ensure continuity of care after hours.

Quality Assurance & Compliance

• Schedule only compliant caregivers with current credentials, background checks, and required in-services.

• Maintain accurate caregiver availability, assignment, and attendance records.

• Partner with HR and clinical teams to monitor and maintain caregiver compliance.

• Support ongoing quality assurance efforts to uphold the company’s service standards.

What You’ll Need

• Bilingual, English Spanish. Preferred but not required

• Associate’s degree or a minimum of 2 years’ experience in home care coordination, staffing, or healthcare administration.

• Excellent verbal and written communication skills.

• Strong computer and organizational skills with great attention to detail.

• Valid driver’s license and reliable transportation.

• Positive attitude, customer-service mindset, and commitment to seniors’ dignity and well-being.

• Three professional references (two supervisors and one co-worker).

• Ability to pass background and employment verification checks.
