Job summary

UnityPoint Health is seeking a Home Care Coordinator for the PACE program in Sioux City, IA. This full-time role supports scheduling in-home services, liaises with the interdisciplinary team, and ensures care plans are followed for each participant.

Position requires a High School Diploma or GED and a 75-hour CNA training, CPR and Mandatory Reporter certifications, a valid driver’s license, and insurance. Typical hours are Monday–Friday, with rotating weekends and holidays.

Qualifications

  • High School Diploma or GED.
  • Completion of a nursing assistant training program totaling at least 75 hours or equivalent education, training and experience.
  • Maintains CPR status or acquire within 30 days of hire.
  • Maintains Mandatory Reporter status or acquire within 30 days of hire.
  • Completes 12 hours of related in-services per calendar year.
  • Valid driver’s license when driving any vehicle for work-related reasons.
  • Valid Auto insurance.

Responsibilities

  • Coordinate home-care services — Schedule and arrange personal care, homemaker, respite, and other in-home services based on participant needs.
  • Support individualized care plans — Ensure home-care services align with each participant’s care plan and changing needs.
  • Monitor service delivery — Track visits, attendance, missed services, and overall quality of care provided in the home.
  • Collaborate with the interdisciplinary team (IDT) — Communicate participant updates and concerns with nurses, social workers, providers, therapists, and other PACE team members.
  • Coordinate with caregivers and agencies — Serve as a key point of contact for home-care agencies, caregivers, participants, and families.
  • Address participant needs and concerns — Respond to scheduling issues, caregiver concerns, changes in condition, and potential safety risks, escalating when appropriate.
  • Maintain accurate documentation and compliance — Document services and communications in the medical record and ensure care coordination follows PACE policies and regulatory requirements.

Under the supervision of the PACE Clinical Manager, the Home Care Coordinator coordinates home care services (personal care and/or homemaking) schedule to meet participant needs. Serves as a liaison and assists Interdisciplinary team members to ensure participant needs related to home care services are implemented through coordination of care.
