Job summary

Trinity Health PACE is seeking an In-Home Attendant to join our caring team in Massachusetts. This full-time role supports participants with personal care, daily living activities, and health status monitoring in home and center settings, following center policies.

You will provide nursing support as assigned, ensure safety, assist with meals and transportation, and maintain accurate written records. A CNA is preferred but not required; some travel and mobility are involved.

Qualifications

  • High school diploma or GED required. Active certified nurse assistant in the state where PACE Organization is located preferred but not required.
  • Minimum of one year of documented experience with frail or elderly population; if less than one year, training provided by PACE on hire.
  • Experience in long-term care or community-based geriatric program preferred.
  • Own insured vehicle for transportation.
  • Must be able to lift at least 20 pounds; frequent kneeling, bending, reaching, walking; fine motor dexterity.

Responsibilities

  • Provide personal care assistance to maintain independence in activities of daily living (ADLs).
  • Provide nursing care as assigned safely per center policies.
  • Identify safety hazards and respond to emergencies; ensure participant safety.
  • Offer behavioral supervision and reminders for nursing and medication instructions.
  • Assist with meals, recreation, and housekeeping tasks.
  • Report signs of physical, emotional, or behavioral changes to staff.
  • Maintain infection control and PPE compliance; follow safety standards.
  • Escort participants to medical appointments and assist in transportation as needed.
  • Attend staff meetings and document care plans; maintain confidential records.

Pay Range: $17.11-$20.10/hr

Sign On Bonus: $750

Position Purpose:

The In-Home Attendant is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.

What you will do:
  • As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming. Assist in the maintenance of physical activity, exercise, and walking.

  • Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the center.

  • Responsible for participant safety, identify safety hazards and emergency situations, and initiate corrective action as able.

  • Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.

  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.

  • Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.

  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.

  • Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PACE Organization vehicles as needed.

  • Attend staff meetings and participant care planning on a regular basis.

  • Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participants expressed wishes.

  • Prepare and submit timely written reports as required. Participant assistance may be required in the center, clinic, or home health/In Home Services department.

  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.

  • Maintain strict and confidentiality of all participant information

Minimum Qualifications:
  • High school diploma or GED required. Active certified nurse assistant in the state where PACE Organization is located preferred but not required.

  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

  • Experience in a long-term care facility or with a community-based geriatric program preferred.

  • Own insured vehicle for transportation.

  • Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.

    • Physical mobility that includes movement from place to place.

    • Must be able to sit for long periods of time.

    • Physical agility, that includes ability to maneuver body while in place.

    • Dexterity of hands and fingers.

    • Coordination, including eye-hand, hand-foot.
