Job summary

UnityPoint Health seeks a Home Care/Hospice Aide to deliver high-quality patient care in home settings under RN supervision. Responsibilities include daily living assistance, light housekeeping, and companionship, with documentation and adherence to care plans.

Qualifications

• Completion of a 120-hour Nursing Assistant course.
• High School Diploma or GED required.
• Must be at least 18 years old per regulatory requirements.
• 2–5 years of home care or hospice experience preferred.

Responsibilities

• Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility.
• Provide light housekeeping, meal preparation, and companionship.
• Support medication administration and simple procedures under RN guidance.
• Observe and report changes in patient condition or home environment to the RN Case Manager.
• Maintain accurate documentation and ensure compliance with care plans and regulatory standards.
• Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives.
• Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries.

The Home Care/Hospice Aide delivers high quality patient care in the home environment under the direction of the RN Case Manager. The Home Care/Hospice Aide provides personal care, homemaker services, and companionship to patients and ensures that high standards of care are met at all times.

Education

• Completion of a 120-hour Nursing Assistant course.
• High School Diploma/GED High School diploma or GED

Experience

• Must be at least 18 years of age in accordance with regulatory requirements Required.
• Completing of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CAN work after completion
• Required 2 years 2-5 years of home care or hospice experience

License(s)/Certification(s)

• Valid Driver’s License (IA, IL, or WI) – Required upon hire
• Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)
• Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred
• Child Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
• Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
