Job summary

Family First At Home in the Renton/Greater Seattle area is hiring a Home Care Client Manager to oversee client care, coordinate with families, and ensure high-quality, compassionate service for seniors at home.

The role is in-office with local travel, full-time M-F 8:00AM-4:30PM, plus on-call rotation. Beneficial CNA or HCA certification and experience in care coordination are valued; onboarding bonus of $1,000 after 60 days is offered.

Qualifications

• 2+ years of experience in healthcare care coordination or related role.
• Valid Driver’s License and vehicle insurance with clean DMV record.
• CNA or HCA certification preferred.
• Proficiency in healthcare management software preferred.

Responsibilities

• Conduct in-home or virtual assessments to identify client care needs.
• Oversee client care and maintain communication with families.
• Coordinate with Caregiver Success Manager to meet client needs.
• Regularly communicate with clients, referral partners and families.
• Maintain accurate documentation of client services and care plan changes.
• Ensure compliance with regulations and internal policies.
• Occasionally participate in community outreach with referral partner relationships.
• Travel within the Renton/Greater Seattle area as needed.
• Provide in-home support to cover for caregivers on call; participate in on-call rotation (one night per week).

About The Role

As a Home Care Client Manager, you’ll be at the heart of our care delivery team—ensuring every client receives personalized, high-quality service. You’ll oversee client care, maintaining communication with families, and providing compassionate service to our clients.

The ideal candidate for this Home Care Client Manager role is someone with a client-first mindset, strong interpersonal skills, and the ability to prioritize effectively in a fast-paced environment.

Job Duties

• Conduct in-home or virtual assessments to identify client care needs
• Ensure client care needs are met, coordinating with Caregiver Success Manager
• Regularly communicate with clients, referral partners and family members
• Maintain accurate documentation of client services, progress & care plan changes
• Ensure compliance with all relevant regulations, internal policies
• Occasionally participate in community outreach with current and potential referral partner relationships
• Travel within the Renton/Greater Seattle area as needed or assigned
• Occasionally provide in-home support to clients, covering for caregivers who call-off, when necessary
• Participate in regularly scheduled on-call rotation handling off-hours staffing needs Your work time is paid and in addition to your standard schedule. Approximately 1 night a week.

Qualifications

• 2+ years of experience in healthcare care coordination, client management, and/or related role with transferrable skills required
• Valid Driver’s License and vehicle insurance with clean DMV record required
• Current CNA or HCA certification preferred
• Proficiency in using healthcare management software preferred
