Job summary

The Hampton-Newport News Community Services Board is seeking an Intensive Care Coordination (ICC) Case Manager to provide comprehensive care coordination for children and adolescents with complex service needs. You will work with youth, families, schools, service providers and community partners to ensure services are coordinated, timely, and responsive.

The position requires a Bachelor's degree in a Human Services field and two years of relevant experience in child mental health case

Qualifications

• Bachelor's degree in a Human Services field (Social Work, Psychology, Counseling) or equivalent.
• Two years of experience providing mental health case management or care coordination for children.
• Knowledge of DSM-5 diagnoses and care management principles.

Responsibilities

• Complete clinical, functional, and service needs assessments and assist with level-of-care determinations.
• Develop and monitor individualized service plans based on the needs and goals of the child and family.
• Coordinate referrals, services, authorizations, and community resources across multiple providers and systems.
• Monitor progress, maintain timely and accurate documentation, and adjust services as needs change.
• Advocate for children and families and provide coordination and support during crisis situations as part of a multidisciplinary team.
